The start and end of every school year are important milestones so I wanted this sheet to include the most relevant information and prompts to capture our kids’ personalities.
I’m sure all children will naturally find a way to make this interview sheet special, whether it’s something funny, just plain adorable, or both!
🖼️ All About Me
This section captures a snapshot of your child’s world at this very moment. From their name and age to their height and shoe size, it’s all about documenting who they are right now.

Also there are 2 prompts:
- “I’m good at…”

and a photo area where your child can add a photo of himself.
For the photo section, consider having your child hold up their favorite book or a drawing they made during the school year. It’s a fun way to add context to their picture.
